payable to Alley Animals, Inc. and mail to:

(Please mark PET TAGS in big bold letters on the envelope)

Include your address and phone number and we will send you the application for the tags. After completing the application just mail it to the Maryland MVA with the remaining $25.00 fee. (A total cost to you of $50)
(MVA's address will be on the application)

Even if you have renewed your registration recently, you can still apply. You will receive your Pet tags with duplicate month/year stickers by mail.
